The UMACO Diabetes Management and
Education Program is Now Accredited

The Diabetes Management and Education Program (DMEP), presented by United Medical Accountable Care Organization (UMACO), was recently named an accredited diabetes education program by the American Association of Diabetes Educators (AADE), a National Accredited Organization (NAO), certified by the Centers for Medicare and Medicaid Services (CMS). This will allow the people with Diabetes in Delaware increased access to high quality diabetes education services.

“UMACO’s Diabetes Management and Education Program is exactly the type of program we envisioned when we set up our accreditation in 2009.” said Leslie E. Kolb, RN, BSN, MBA, Accreditation Director for the Diabetes Education Accreditation Program. “AADE’s accreditation assures that an accredited program meets the National Standards for Diabetes Self-Management Education and Support.”

The UMACO Diabetes Management and Education Program Group Sessions Begin on May 23, 2016 in Bear, Delaware

Effective May 23rd 2016, the UMACO Diabetes Management and Education Program will start its first 8-week long group/class sessions at the United Medical Business Office’s Conference Room in Bear, Delaware. Our future plans include, for the convenience of our patients, sessions to be held at the American Surgery Center in Wilmington, DE – dates and times to be determined.

The program will be taught by Amy Wilcoxon, MA, RD, LDN, CDE and Tim McHugh PT, DPT, OCS, Cert MDT.

The 8 week program includes education on:

• Introduction to Diabetes (Glucose, Insulin, Type I and Type II Diabetes)
• Nutrition
• Being Active – Exercising with Diabetes (presented by Tim)
• Monitoring blood sugar levels
• Reducing risks
• Diabetes Medications
• Symptom Identification/Prevention and Treatment for Hypoglycemia and Hyperglycemia

The program is available to all United Medical Network patients. Patients can continue to be identified and referred into the program by their specialists and primary care physicians.
