UMACO Fall Prevention - Balance Program

Effective August 1, 2016, United Medical ACO (UMACO) will officially begin its Fall Prevention Balance Program, available to all United Medical Network patients. The program is ultimately focused on prevention and better health for Medicare patients 65 years of age and older who are at risk for falling or have previously fallen.

Patients can be identified for the program based on their Morse Fall Risk assessment score and/or Conley Fall Risk assessment score. Through PowerChart reporting and diagnosis, physicians can report the patients whom are at risk for falls or who have fallen in the last three months. Patients will then be ordered for a Physical Therapy Consultation with SOS Physical Therapy where they will undergo a physical therapy evaluation, and an individualized exercise program will be developed; this includes balance, strength, and gait training. The length of the program will be determined based on the patients’ individual needs and risk for falling. Patients’ risk for falling will be reassessed regularly over a 6-12 month period.

Tim McHugh, PT, DPT, OCS, Cert. MDT will conduct patient evaluation and treatment at current SOS Physical Therapy locations in Newark, Bear, and Wilmington, Delaware.

Patients can continue to be identified and referred into the program by their specialists and primary care physicians.

As a graduate of the University of Maryland, Tim received his Doctorate in Physical Therapy from Elon University in 2009. After graduating, Tim worked in an outpatient orthopedic setting treating a range of musculoskeletal conditions for four years prior to joining SOS Physical Therapy. In 2013 he became Board Certified in Orthopaedics through the American Physical Therapy Association (APTA), as well as a certified clinical instructor.
